Dear Health Care Provider, Your patient is a student-athlete planning on or participating in intercollegiate athletics at Southeastern Louisiana University. The National Collegiate Athletic Association (NCAA) bans the use of some stimulant medications unless medical necessity is clearly documented by the university. Stimulant medications utilized for the treatment of Attention Deficit Hyper Activity Disorder (ADHD) and Attention Deficit Disorder (ADD) fall into this category and require documentation of diagnosis and treatment. The documentation that is needed will assist in the case a drug is detected in their urine during a drug test and a request for medical exemption is required to be submitted to the NCAA for the student athlete. Southeastern Louisiana University Sports Medicine is requesting the following information in order for your student-athlete to continue or begin their NCAA intercollegiate athletic participation (this is critical for their participation). If the athlete is new to Southeastern Louisiana, please attach documentation that may clarify the studentathlete's diagnosis and the need for them to use the stimulant medications for treatment along with the forms enclosed. Returning athletes are required to submit the form enclosed annually if they continue to participate in NCAA athletics and require stimulant medication for their treatment. By completing this paperwork, you acknowledge that you have reviewed the patient's health history and have informed them of the safety information regarding stimulant use.
